Registration Information
All of the following requirements must be complete before registering a student.
Please bring the following with you:
Birth certificate
Immunization/Shot Record

DPT series with last booster after 4th birthday

Polio series with last booster after 4th birthday

Measles, Mumps & Rubella (MMR) given after the first birthday - 2 doses

HIB given after 18 months (required for Pre-K and Kindergarten)

Hepatitis B - 3 doses

Varicella (Chicken Pox) - 1 dose (required for Pre-K and Kindergarten)

List of Prescription Medications if applicable
Proof of Address
Your lease/mortgage agreement, a light, telephone or other bill - within the last two months.

Age Requirements: Complete School Choice Application in the HISD website

Pre-Kindergarten - 3 / 4 yrs old on or before 9/01

Parent Drivers License or Identification Card (I.D.)
